Referring Physicians:

If you would like to refer a pregnant patient please send your referral to Dr. Carnevale Lizotte at the Cobourg Medical Centre at the contact information below. Please include any prenatal bloodwork or ultrasounds completed. Please note, we will send the mother and baby back to you after they have completed their course of care and maternity services will not affect FHO services at your location.

*Referred patients will be scheduled for an initial consultation within 2 weeks.
